Rhinoplasty surgery is an operation of the nose, which is done to change the appearance and structure of the nose. SURGICAL PROCEDURE OF NOSE RHINOPLASTY SURGERY/ NOSE JOB PATIENTS – A complete medical history is taken; Entire face is evaluated for shape, size, symmetry and proportion including the chin. Next a systemic assesment of the Nose is done, in the following way -- Nasal tip (size, shape, position of tip defining points, skin quality like thickness ), Nasal base & Ala, Columella ( position in the midline, straightness, width, length, position relative to alar rims ) Cartilagenous mid vault, Nasal Bones (length, width of the bony pyramid, junction of the upper lateral cartilage & nasal bones), Profile (nasal length, height, contour, tip projection, nasolabial angle) ; Careful examination of the septum, turbinates, Internal & External valves ; High quality photographs (front view, worm eye, lateral views ) are also taken. Some of the above examination may need cooperation & corelation with an ENT specialist, to analyse the functional aspect or for management of Internal pathology, which may affect breathing like –Deviated nasal septum, Nasal Polyps, Hypertrophied turbinates. A smoker should not be operated upon, unless he is ready to stop smoking before & after nasal septum deviation surgery. All required investigations for Clearence for surgery, under general anaesthesia should be done.

SURGERY PROCEDURE - As mentioned earlier, Nose cosmetic surgery is mostly done under General Anesthesia, for patient comfort and cooperation. A transcolummelar incision is placed, in the open approach, in such a manner; that after healing it is barely perceptible, when seen by onlookers. Infracartilagenous extensions are then performed bilaterally; along caudal border of lower lateral cartilage. These incisions meet the transcolumellar incision, to complete nose repair. After that skin envelop dissection is done very carefully to raise it up,and to expose the underlying osteocartilagenous nasal framework, so the cartilagenous dorsum, lower and upper lateral cartilage, and bony dorsum is clearly visible. Afterwards the selected area for improvement is addressed by appropriate surgical manoeuver. Sometimes cartilage graft is reqired for Augmentation/support of a particular area. This is obtained from Either Nasal Septum or from the Ear.



POSTOPERATIVE INSTRUCTIONS AFTER RHINOPLASTY RECOVERY -- Patient can be discharged a day after nose repair. During the first 48 -72 hours, the patient is instructed to keep the head of bed elevated at 45 degrees and use a chilled gel eye mask to help minimise post operative swelling. Patient are kept on a liquid diet on day of nose augmentation surgery and then shifted to soft regular diet the following day. Any foods that require excessive lip movements, such as eating apples or corn on the cob, should be avoided for 2 weeks after nose job. Nasal pack given at time of surgery is removed 2 days after surgery. Nasal Splint and sutures are removed in the next follow up, done after a week. During this interval patients are kept under cover of antibiotics and pain killers. Other medications like decongestant & drugs to reduce swelling are also given. The swelling takes some time, to settle down and patient has to keep patience during this interval bleeding, infection, delayed healing can sometimes occur.
